Report a Grievance / Customer Complaint

To file a grievance or complaint about a product, please email the following information:

Subject Line: GRIEVANCE DEPARTMENT

 

1. Your name, legal address, and contact information.

2. Nature of your complaint (including product or service).

3. Product Specifications (Product name, ID, detailed photographs of product labels).

4. Purchase receipt or other form of record showing proof of purchase, including date.

5. Origin of product purchase, including name and contact information of seller.

6. Detailed description of your complaint.

7. Date product/service was used after purchase.

8. Any additional supporting documentation of your grievance.
